Free Flu Vaccines Eligibility

You are eligible for a free flu shot provided you are a Medicare Card holder and meet the following criteria:

  • Aged 65 and over
  • Pregnant women
  • Children under 5 years old
  • Aboriginal ethnicity
  • Medically at risk groups (Chronic conditions such as diabetes, asthma, lung conditions, disease of the nervous system, impaired immunity, kidney disease, etc.)

Appointments

Cronulla Street Medical Centre runs on an appointment system. You can make an appointment by contacting the practice by phone or booking online.

  • Face to face appointment, which can be standard or long appointment
  • Telehealth ( Phone) appointment, which is available as standard appointment to eligible patients
  • Walk-in appointment:  Walk-ins are subject to availability on the day

Cronulla Street Medical Centre aims to adhere to appointment schedule. However, our doctor may sometimes run behind the schedule due to unpredictable nature of medical practice.


Home Visits

Our medical centre offers home visits to our regular patients at doctors’ discretion. Please enquire reception for fees and availability.


Reminders and recalls

Our medical centre routinely reminds patients about health services and appointments (via
SMS, letters or phone calls).

Please update us with your correct contact details.

If you do not wish to participate, please let us know so that we can respect your privacy.  


Communications

Telephone communication

We are not always be able to forward your call to our GPs. However, we are happy to take a message and return your call if medically necessary or as per doctor's discretion. If you need to speak to a doctor, please book a standard appointment or a telehealth appointment. 

Email communication

Our emails are not monitored regularly and should not be used to contact the doctors for any clinical matters. Clinical information is not shared via email unless patient consent has been provided at their own responsibility. All emails are read by admin team and distributed to the correct member of staff such as your doctor, the practice nurse or management.


REsULTS, SCRIPTS AND REFERRALs

In order to monitor your health, fulfil our ethical and legal responsibilities, you will need to book an appointment to receive results, scripts or referrals. Our receptionist are unable to provide you with any results or information related to results. 


Transfer of Medical Records to Another Practice

We must receive a Transfer of Medical Records request on the letterhead of the business requesting it. Each request must be signed by the patient. Please enquire reception team regarding requirement and fees for transfer.

We welcome any feedback about your views of the practice and how we can improve your care and service. Your feedback is completely confidential.

If you are unhappy with any aspect of the care you receive at this practice, please let us know directly or anonymously as you prefer.
